Our goal is to deliver an exceptionally friendly, prompt and professional service providing you with the best in physiotherapy care. Our experience tells us that there are some key areas we need to focus on to ensure that you receive the greatest benefit from our services.

 

Recovery: Remember that healing and recovery takes time and not everyone heals/recovers at the same rate. If you do not feel that you are responding as well as expected during your care, we would ask that you discuss this with your practitioner/therapist. We want you to get the most from your care at Physio4Life.

 

Appointment Scheduling: Your physio will outline the best-recommended action plan for your injury. You will achieve the maximum results when you keep your recommended action plan to this schedule. Therefore, to receive the most out of your care and to save time we advise that you schedule your appointments in advance.

 

Missed Appointments: Missed appointments will set you back in your recovery. If an appointment needs to be changed, we require 24 hours’ notice. All missed appointments must be made up later in the same day to avoid a cancellation fee of 100% of your appointment cost. The fee is not covered by compensable bodies and must be paid by the patient. People who repeatedly miss or reschedule appointments will regretfully be discharged from care as we realise you will not reach your health goals and we do not wish to waste your time.

 

Fees and Your Account: Fees for all patients are due at the time of service. All our physiotherapists are chartered and registered with HPC as well as recognised by major health insurance companies. Unfortunately, we do not hold direct contracts with all insurance companies other than IPRS, BUPA and AXA PPP, but we are recognised by most major insurance companies. If you have private, medical insurance please speak to the receptionist, as we will need to take some further details.

 

Mandate Form: Physio4life can invoice directly to our contracted Insurance Companies and Insurance Companies that we are recognised with on your behalf. In order for us to process your invoices, we ask that you complete a copy of our mandate form to ensure that the clinic will be covered for any shortfalls and/or excess on your membership/policy with your insurance. Any shortfalls/excess must be paid directly to the clinic as your provider. We are unable to invoice directly to your insurance company if a mandate is not completed.

 

Prepaid Sessions:  Any prepaid courses for Personal Training, Pilates and Massage are valid for six months from the date of purchase. Monies cannot be refunded or transferred to other services at Physio4life.

 

Promotions and Offers: Pre-payment is required for all promotions and offers at the clinic. They cannot be used in conjunction with any other offers at the clinic. Some promotions and offers do not fall into our 24hr cancellation policy. This is because the offer/promotion is dedicated to a specific day, and so cannot be used on an alternative day. Please make sure you read the terms and conditions of every promotion/offer.

 

Data Protection & Privacy Policy: Physio4life need to collect personal information as a medical provider. We comply with the GDPR (General Data Protection Regulation)– That all personal information collected is stored safely and not disclosed unlawfully. You can obtain a copy of your medical records by making a subject access request. You can access Physio4life’ full Data Protection & Privacy Policy on our website www.physio4life.co.uk or request a hard copy from a member of staff.
